Clinical Case Manager
As a member of the Clinical Team, perform administrative duties in support of residential and therapeutic services for adults with SMI, TBI, and dual diagnoses. Support the tracking of documentation and scheduling of staffings to contribute to an environment which promotes growth for each member of our team and everyone in our care. Support a clinical team of Counselors and medical records, to ensure the goals and vision of Orion Homes meet the needs of all clients.
Duties include:
- Serve as part of Orion’s clinical team.
- Schedule and facilitate treatment team meetings and program visits with outpatient clinical teams and family members. As needed.
- Maintain communication, coordination, and connections with all clinical team participants.
- Track when documentation is due and coordination of signatures.
- Track documents that have not been turned in by House Managers.
- Track completion of Group Facilitator Note submissions via Excel.
- On an as needed basis, support Counselors with scheduling of monthly Staffing’s
- Maintain the residents’ Contact List and Tracking Spreadsheet.
- Ensure Incident Reports are reviewed and sent out to the Care Team.
- Create new client folders and templates.
- Conducts intakes with incoming clients
- Evaluate intake documents (e.g., psych, case management, doctors, and other medical sources) to determine eligibility for placement
Knowledge, Skills & Abilities:
- Strong organizational skills.
- Knowledge of laws and ethical standards relating to the care and assistance for individuals with SMI, developmental disabilities, and other disorders.
- Knowledge of principles and practices of case management.
- Skills in problem identification and resolution.
- The ability to follow direction, use of sound judgment and excellent time management skills are essential.
- Must maintain good recipient relationships, be able to identify priorities and stay organized.
- Knowledge of community-based behavioral health services.
- Comfortable interacting with SMI community
- Must have communication etiquette via email, phone, and video chat.
- Must be proficient in Excel, Outlook, secured emails, and have knowledge of or skill with current technology.
- Strong knowledge in HIPPA compliance
- Knowledge of SMI medical terminology
Qualifications:
- Minimum High School Diploma
- A minimum of one-year experience providing case management services
- Ability to acquire a Level 1 Fingerprint Clearance Card, without restrictions.
